When the Autism individual gets a customized person-centered plan, it is important to have expertise from the multi-disciplinary team or from the specialist service, which can be related to the developed or action as per the person-centered plan. Such as, if there are any aim and goal to make the autism person develop, learn and have an in-depth communication, provision of assistive technologies or accessibility (Dehlendorf, 2016).

With the help of the multidisciplinary team, it has also created a uniqueness for the individual at the centers of collaboration and getting supporting circle members. Through the multidisciplinary support and the interagency approach, it helps to create independent choices and goals. At the same time, it can help in creating an exciting and customizing person-centered planning which can help an Autism individual to attain necessary support and also get ways through which it can help to accomplish their goals
Through the support of staff within the person-centered planning, it helps to create a powerful possibility to attain an outcome accomplishment. Three models of practice
As per the practice of the problem-solving exercise, it is more focused on the right to access resources and how to ensure empowerment not creating a sufficient one. As observed how to build a conscientization process having the empowerment creating an awareness-based to the right to access existing resources, but of the right and responsibility to participate in creating resources, and eliminating inappropriate or ineffective resources" (Breton, 1994, p. 29). Such consideration of responsibility for social changes is essential to all progressive social workers and is consistent with the literature on planned social change. The goal of the social worker is to provide quality person-centric planning should maintain and strengthen relationships with family. Due to the empowerment theory, it can help to relate to the planned social change having a certain focus. Based on the problem solving and having a conscientization (it would create a more personal along with the structural dimensions based on the required situations or problems). It can also create an empowering strategy based on the cognitive restructuring which can help in evaluating to seize or create opportunities based on the environment related to the structural dimensions which can help gain an advantage (Holland, 2016). It includes and relates to the resources which can help in creating a will or motivation based on seizing opportunities.
When relating to the empowerment practice it is important to relate with the power relations. Firstly, it would help in creating an exercise power, and it can help in relating to three kinds of authority, which is not attributed the same to exercising it. It is also noted as the power latent. Secondly, it is also related to how to place the exercise of power with the social relations (Jansen, 2017). In case the person has no family or any other involvement, then the social worker has to prioritize and create social relationships at every stage of relating it with the person-centered planning. Such social relations would consistently be related to empowerment—and having a structural condition that would not be affected due to the power of allocation and including an opportunity to relate with the environment. Lastly, how empowerment is related to the power relations which would follow a symmetrical or asymmetrical. It is accounted for relatively equal amounts and relates to power and authority which can be based on reciprocity (mutual influence). It is related to an unequal amount creating power and authority, which can help to manage in terms of subordination along with the superordination. The power of empowerment results in individual enlightenment and relates to the abilities and needs, result "empowered". To ensure how an autistic person would help to attain would abilities and needs help and labor and social inequality.
Relating to the power of the real commitment of emancipation helps the Autism individual to relate with the self-actualization within the social environment (Strasser, 2018). To make the individual more involved in the person-centered planning along with the social worker from the beginning. At the same time, having an emancipation practice is more related to how and why the world is built in the new way of the world. To understand emancipation it id more related to visionary ideas and having a moral ideal. The practice helps to relate with the social workers and helps to relate with the vocation or "calling, other the" rather than "job,", At the same time having an immediate satisfactions and dissatisfactions, can also be attributed to the desired values. Through emancipation and the empowerment practice, it can help to create individual enlightenment and make the social worker instill a collective action and relate with the social transformation along with structuring with the new arrangements. The role of the social worker is to relate with the collective action, having an agency as per the community problems and relate with the problem-solving.

The practices and theories are linked with the patient centric care plan and needs to be monitored and relate with the goals outcomes. Due to the Autism, it can cause a problematic concerns and show the delay signs and symptoms, such as the impaired or a low social interaction; having an impaired or the limited language or the attainment of the communication skills; along with gaining an unusual patterns related to the thoughts and physical behavior.
