What is being submitted: 4000 word assignment
Assessment Description
The assessment for this module is a:
4000 word written assignment, to be submitted using Turnitin by 2pm on the date stated by the academic office.
In the assignment you will select one service user/ person from your practice area, on whose care you will focus. In this piece of work you will outline, discuss and analyse the assessment and planning of their care. To achieve the depth of theory that is required it is expected that you will choose and use repeatedly and throughout the discussion a model, framework or approach to care which will inform, and be recurrent throughout your discussion (see the formative assessment). To clarify this point, you may use: a nursing model, a theory about health/ healthcare, a philosophical approach or a framework. Only choose one, and provide a justification, rationale and explanation of your choice of approach.
The 4000 words of the assignment includes all of the text of the assignment. Words in the headings, the reference list and appendices are not included. It is expected that you will not exceed the word limit, though you may submit a piece of work below the word limit. However, to cover all of the required areas of information in sufficient depth it is unlikely that you will submit an assignment that is comfortably within the word limit.
Unlike in practice, where in assessment there is an emphasis on “seeing everything” bear in mind that for this exercise we are not looking at how much or how widely you can assess, but instead encouraging you to reflect on what is assessment and care planning, what these concepts mean, and how in practice we apply these notions using certain specific skills. Where you choose to describe specific aspects from the case there should be reasons why, and an intended purpose for your choosing these particular excerpts. In the discussion of the planning of care we do not require that you produce an all embracing plan of care covering the person’s every need or area for development. Instead, you will need to make choices and demonstrate your skills of identifying the most important aspects of the practice based case through logically choosing content to discuss and developing a line of argument that is logical and well supported by the literature.
Follow the guidance below in structuring your work, and use the suggested headings as a means of ensuring that you cover all of the areas of information that are required in a balanced manner. Please note that marks will be lost for not following the criteria below, and give particular attention to the comments in bold type font:
Save your assignment and upload it on Turnitin using your SID number, so that your marker will clearly know the reference code of the script. In addition to this, place your SID at the top of the first page of the assignment. Do not put your name anywhere on the assignment, and avoid including the name(s) and personal details of any other person in your work. It is acceptable to use “I” because this is your piece of work and this indicates ownership and is more natural. Using the third person pronoun “the writer suggests…” requires the use of more words and is awkward. However, if this is your preferred style of writing this is acceptable.

Biography
With reference to the case of dementia, it is evident that I was placed in a specific clinical area, where I would be enabled in demonstrating my skills and expertise in nursing. As I enriched with the skills of mental health nursing and adept in dealing with the patients diagnosed with dementia, I was allocated with the task of treating a patient of such kind. After thoroughly checking and glancing through patient’s biographical and other medical history related details, I have come to learn that patient, 65, has been diagnosed with a wide range of symptoms, such as forgetfulness, limited social skills and impaired thinking abilities. By profession, he was a professor of an UK-based college and he taught History. At the age of 60 years, he was retired from his service. His family is nuclear and as far as his friends are concerned, they are limited in number. His name is Abraham Collins and he graduated and mastered from the University Cambridge. He is not married and he lives in his London-based residence with his mother, father and brother. His parents were also graduated and his brother acquired master degree from the University of Sussex. For entire three generations, Collins family has been serving the British Army. However, it has been traced that Collins family has a demonstrated history of being diagnosed with dementia, which aroused my suspicion in me that Abraham must have had genetic reasons for being diagnosed with Dementia. At this point of time, I have deciphered certain symptoms in him, which can be equated with Dementia, such as loss of memory, decline in mental health and lack of orientation. In addition to these, Abraham Collins has been suffering from other symptoms because of being affected by Dementia, such as lack of capability of understanding language and lack of ability of recognising common things. Because of the occurrence of these symptoms, it is clearly evident that Abraham is diagnosed with Dementia. There are certain things, which Abraham Collins experiences for being a patient with dementia, such as behavioural, psychological, muscular and other common facets. With regards to behavioural aspects it can be notified that the patient has been experiencing issues when it come to combining muscular movements. A wide range of theoretical narratives have been put forward concerning mental health Nursing. American psychiatrist Sullivan introduced the theory of interpersonal relations in accordance with which mental health diseases are manifested because of interpersonal experiences. Therefore, it is important to see that interpersonal experiences are rehabilitated and at the same time, positive relationships between mental health nursing and service users are nurtured. From this perspective, Pehlivan and Güner (2016) and Crowe et al. (2008) placed emphasis upon the adoption of interpersonal approach and effective techniques for relieving stress as well as anxiety. With the help of implementing this theory, it can be possible for nursing professionals to ensure that positive relationship between nurses and patients. On the other hand, there are developmental theories, insemination is the initial phase of human development and it continuous through different phases of lives. In this context, narratives having relevance with psychoanalytic theory can be mentioned along with Freud’s psychosexual development theory. In accordance with these theoretical narratives, emphasis is placed upon problems’ resolution approach and its application for managing service efficiency. From this perspective, it can be said that nurses can provide all kinds of supporting facilities to patients in times of their needs and the very idea that coping mechanisms served childhood purposes are effective during adulthood, is abstract. In light of Erikson’s developmental theory, it is important to inform patients’ relatives and parents concerning treatment procedures and at the same time, their permission ought to be acquired before commencing the process of treatment.
In addition to these, there are other theoretical narratives, such as humanistic theories, biological theories and social theories. These theories have been applied in a different manner while addressing the roles of mental health nurses addressing mental health issues. In accordance with humanistic theories, safety of patients must be given priority for the sake of enhancing healthcare services efficiency level. In accordance with Maslow’s law, it can b said that it is important for nurses to understand the significance of adhering to physiological and psychological requirements of patients. Linking Assessment of Person with Particular Theoretical Evidences
On the basis of critical analysis of the case of Abraham Collins, it is possible to adopt behavioural theories. During interview only, two people were present there, the nursing professional and Abraham Collins, who was a mentally ill patient diagnosed with dementia. I carefully selected facts and evidences from the assessment for emphasising and being aware concerning various aspects of behavioural patterns of the patient diagnosed with dementia. While selecting facts and evidences from the assessment, scholarly and academic evidences were used and at the same time, fresh data were collected from the respondents through interview.

Like other sectors, communication has become a matter of prime importance in healthcare setting. In the setting of mental healthcare, the application of communication is of prime importance. In accordance with the opinion of Teutsch (2003), it is necessary to ensure effective verbal and non-verbal communication in healthcare settings so that healthcare related ideas and principles are effectively communicated. Through effective communication, it can be possible to maintain therapeutic rapport can be developed and at the same time, it can be possible to dealing with issues in healthcare settings. At the same time, it is important to remember that the engagement of patients and staffs is required for generating required productivity. In this context, it can be said that engagement is of prime importance in healthcare setting. In the context of healthcare setting, it can be said in the light of the opinion of Polacek et al. Through enhancing patients’ awareness about health, nurses are enabled to keep them engaged, whereas nurses are kept engaged though ensuring effective communication and collaboration along with their involvement in decision-making process. Through facilitating engagement and communication in mental healthcare setting, the application of social theories of healthcare is ensured. However, there are certain barriers to communication in mental healthcare settings, such as lack of collaboration and communication effectiveness along with the lack of rapport amongst nurses. These problems can effectively be handled with the help of implementing effective communication policies. At the same time, communication mechanisms can also be used for demonstrating positive attributes of organisational healthcare philosophy. However, there are certain risks, which are faced in healthcare settings, such as cyber risks, possibility of occurring violent incident and increase in malpractice litigations. With reference to the person, it can be said that risks factors may occur in case appointed nurse is unable to maintaining communication with him. For dealing with these risks, it is important to introduce effective risk management and risk mitigation plans. At the same time, it is important to see that all these risks can be identified and ranked as per the fundamentals of risk prioritization.
Care Planning
Scholarly Insight into Narratives of Care Planning
In mental healthcare setting, care plans and care planning carry a greater degree of significance and their principles are effectively applied for the long-run. In accordance with the opinion of Burt et al. (2014), patients and healthcare service provides do not have clear idea concerning the formulation and application of care planning and at this point of time, it has become essential for nurses to develop an action plan for delivering care planning to people requiring mental healthcare services. Referring to widespread policy interests and empirical research, it can be notified that assessing nature of care planning and plans can be useful for the improving overall healthcare efficiency level. Application of Care Planning with reference to Case
Abraham Collin’s personal care was planned effectively by adhering to a structure format of activities. Before commencing the process of making planning of care for dementia-affected patient, I had to take many things under consideration, such as likes and dislikes of the patients. In addition, I had to include many activities into daily plan during different phases, such as morning, evening and night. In case, he gets bored, I, as a mental health nurse, used to encourage him to be involved into daily activities. In accordance with the opinion of Clarke et al. (2009), goal setting the interventions contribute to ensuring the improvement in quality of goals in the setting of mental health services.
